Health and care organisations across north central London are working together to join-up health and care records across our five boroughs
This work will result in an integrated electronic health and social care record that will help to improve the quality of care for our residents.
The record two elements:
A summary of our residents’ health and social care information together in one easy-to-view real-time record, that can be accessed by a resident’s care team by clicking an icon within existing clinical and social care systems.
This will be facilitated by a software application called Health Information Exchange (HIE).
An increased collective ability to be more proactive in the care of our communities. The system will take elements of health and care information from different sources and enable us to care for groups of residents, as well as individuals, in relation to health or social conditions.
This will be facilitated by a software application called HealtheIntent.
This approach, and access to the applications, is being introduced gradually across the five boroughs, starting with a number of early-adopter sites that will launch the HIE shared record by March 2019. HealtheIntent will follow at early adopter sites from Summer 2019 onwards.
Our programme is a step towards creating a cross-London health and care record, as part of One London. The One London model focusses strongly on the delivery of data sharing between providers at the point of care across London to inform and support clinical decision-making and inclusive patient access.
The shared care record will provide immediate access to important health and care information in as near to real time as feasible, at point of care, for the whole care team. Most important information from the main providers of health and care will be real time but in some cases will be updated every 24 hours.
Access to this summary view will save time and support everyone delivering health and care services in a number of ways:
- A snapshot view of a resident’s health and care history, current and past medications and previous events and episodes of care will be summarised
- Information such as discharge summaries, clinic letters and case notes will be available so you will have a broader understanding of a resident’s story
- Current medications, medical history, allergies and information about appointments will be readily available
- Tests and investigations requested by other providers (with results stored in other systems) will be immediately available to you so there will be no need to manually request the results of healthcare tests
- Resident’s will no longer need to retell their story, or recall medication dosages or the outcome of previous care
A recent study of the use of HIE in east London (through the East London Care Partnership) is showing significant benefits for 1.5 million residents:
- 81% of clinicians felt HIE had a positive or very positive effect on their working day.
- 9,400 hours of clinical time have been saved per year, saving an estimated £940k
- Approximately 1,233 referrals have been avoided each year, saving an estimated £133k
- Consolidating systems and eliminating dual entry is estimated to save £500k
The Healthy Wirral partnership recently asked clinicians using the system about the benefits for them, which included saving time and easier access to patient information:
Time spent searching for admission / appointment information per day:
- Time saved each day per user: 12 – 17 minutes
- Extrapolated across Wirral: 58 – 80 hours per day
Time spent searching for pathology results per day
- Time saved each day per user: 6 – 10 minutes
- Extrapolated across Wirral: 30 – 46 hours per day
Time spent searching for imaging results per day
- Time saved each day per user: 6 – 9 minutes
- Extrapolated across Wirral: 28 – 43 hours per day
Change in main contacts for patient information:
- Reduction in contacting GPs and other acute hospitals
- Increase in contact with other services, particularly community services
- Potential time saving enables staff to contact other organisations that aren’t on HIE
How shared records are supporting care
Connecting information with a Health Information Exchange (HIE) has benefits that resonate across the continuum of care. Keeping health and care information safe and ensuring residents understand their choices about how it is used
If a resident decides they don't want to be part of the shared record, they can choose to object and opt-out.
As each organisation prepares to share records they will publish a privacy notice and residents will be informed to via communications materials in local health and care settings affected, which will explain how their information will be used, alongside the benefits. Information on how to object, and opt out, will be included in this.
These materials will be developed in collaboration with resident representatives.
